What to Bring to a Clinic Visit

Before You Arrive

If you receive paperwork in the mail before your office visit, please fill it out before you arrive for your appointment.

We Know Your Time is Valuable

To help you move through the check-in process in a timely manner, please arrive 15 minutes before your appointment and have the following items ready.

Items to Bring

Any required paperwork (if applicable).
Insurance card and photo ID.
Money for any upfront fees or insurance co-payment.
Referral authorization (if applicable).
A current list of medications you are taking.

Need to Reschedule or Cancel?

Please provide 24 hours notice if you need to reschedule or cancel your appointment so that the time can be made available to someone else.
