Physician Orders for Life-Sustaining Treatment – or POLST – have been around for 24 years in Oregon yet many of us do not know about it.

POLST is a simple medical order that outlines the preferred treatment options of a patient with a progressive illness or condition who may be near the end of life.

It provides simple medical orders that can immediately be put into effect by any first responder or clinician. 

It is a brightly colored, single-page standardized form that can be quickly identified by emergency responders throughout much of the United States.

The POLST Registry maintains a secure and confidential electronic record of POLST orders that can be accessed by emergency medical professionals during a crisis, when a paper copy cannot be found.
