Belong Health ACO and Samaritan Health Services will conclude the Centers for Medicare & Medicaid Services ACO REACH program on Tuesday, March 31, 2026.
This will NOT affect your Medicare benefits coverage or your ability to see your doctor.
Participants in the Belong Health ACO REACH program will receive a letter about special ACO-specific benefit enhancements that will conclude, but you will continue to receive care from your primary care provider without interruption, and you may continue seeking services from any Medicare-enrolled provider. Your access to Medicare-covered services will remain the same.

Q. What is ACO REACH?A. ACO REACH stands for Accountable Care Organizations Realizing Equity, Access and Community Health. ACO REACH was a Medicare program designed to help coordinate your care and improve quality. It operated behind the scenes – your provider participated on your behalf to help ensure you received well-coordinated, high-quality care. You may not have noticed any difference in how you received care.
Q. Does this affect my Medicare coverage?A. No. Your Medicare benefits are not changing. You will continue to have the same Medicare coverage you have always had. This transition has no impact on your eligibility, benefits or covered services.
Q. Can I still see my doctor?A. Yes, absolutely. Your relationship with your doctor and care team is not affected in any way. You can continue seeing your current providers just as you always have.
Q. Do I need to do anything?A. No action is required on your part. This transition happens automatically. You do not need to call Medicare, sign up for anything, or change your care in any way.
Q. Will my prescriptions or treatments be affected?A. No. Your current prescriptions, treatment plans, and referrals will continue as normal. There is no interruption to any ongoing care.Q. Will my bills change?A. You should not notice any change in how you are billed. Medicare will continue to process your claims as it always has. Starting April 1, 2026, your claims will be processed directly through traditional Medicare Fee-for-Service rather than through the ACO REACH program.
Q. Will I owe anything because of this change?A. No. There are no costs to you as a result of this transition. Your out-of-pocket costs, copays, and deductibles remain the same under your existing Medicare coverage.
Q. What is Belong Health?A. Belong Health is a healthcare company that partners with medical practices to support patient care through Medicare programs. Belong Health worked with your provider’s practice as part of the ACO REACH program.
Q. Why is Belong Health leaving the program?A. Belong Health made this decision due to structural challenges with the ACO REACH program that affected many participating organizations nationwide. Over30 ACOs are exiting the program in 2026. This decision wasnot related to the quality of care you receive or your provider’s performance.
Q. Will Belong Health contact me again?A. You should not expect further communications from Belong Health. If you have additional questions, your provider’s office is the best point of contact.
