Our team includes nurses and dietitians with special training and certification in diabetes care. They will listen and provide guidance based on your needs and goals. Diabetes Education Offers Counseling for diabetes management and nutrition therapy. Continuous glucose monitoring to help assess blood sugar patterns and guide treatment. Medication management, working with your doctor to adjust diabetes medications to improve blood sugar control. Training in insulin pump therapy – our certified pump trainers can work with you to be successful at wearing and managing your insulin pump. Helping you set goals for changing behavior to improve health. Self-care for Managing Diabetes We’ve outlined seven key self-care behaviors for managing diabetes. Let your Diabetes Education team know if you’d like to dive deeper into any of these during your upcoming diabetes education appointment. Our team is here to provide the guidance and support you need. Healthy coping: A positive attitude toward diabetes and management. Healthy eating: A pattern of eating which includes a balance of nutrient-dense foods to support your health goals. Being active: Structured or unstructured body movement that may support your health goals. Taking medication: It’s important to follow your provider’s instructions when taking prescribed medications. Be sure to discuss any concerns you have about your treatment plan with your health care team. Monitoring: Regularly checking blood sugars and seeing how various factors may impact glucose control. Reducing risk: Naming risks and changing behaviors to minimize complications. Problem solving: Developing strategies to resolve problems or complications. What you should bring to your diabetes education appointment: Blood glucose meter. Food or blood-sugar logs you may have. List of herbs or supplements you may be taking. Nutrition Education Offers Medical Nutrition Therapy for Conditions Including General healthful nutrition for disease prevention and weight management. Metabolic testing is available. Specific dietary patterns, including Mediterranean dietary pattern, DASH diet and plant-based diets. Cardiovascular disease, including high blood pressure or hyperlipidemia. Nutrient deficiencies, nutrient specific diets and navigating a balanced diet with food allergies. (Please note, we do not test for food allergies.) Malnutrition or symptoms of poor appetite or intake. Kidney health (kidney stone prevention or chronic kidney disease, not on dialysis). COPD/Pulmonary nutrition therapy. Metabolic dysfunction or liver disease. Gastrointestinal conditions or symptoms (constipation, diarrhea, IBS, IBD, gastroparesis, celiac disease, ostomy nutrition therapy, pancreatitis, diverticulosis). Pediatric nutrition therapy (general healthful eating, family nutrition education, selective eating). Maternal nutrition therapy before, during and after pregnancy. Please contact your Diabetes Education team for information about other nutrition concerns not listed above. Our Classes Diabetes Self-management: This class is appropriate for anyone who is new to diabetes or if you need a refresher. Content covered will include healthy eating, medication management, blood sugar monitoring, physical activity, risk reduction, problem solving and healthy coping. Nutrition for Chronic Disease Prevention and Weight Management: Learn how to structure meals and choose specific foods for weight management and to prevent diabetes, heart disease, cancer and liver disease. Nutrition – Beyond the Basics: Understand carbohydrate, protein and fat’s role in health. This class will teach you how to select food to reach your nutrition goals. Nutrition Therapy for Kidney Health: Learn how to select and prepare foods that are healthy for your kidneys. This class is not recommended for people on dialysis. A referral from your primary care provider is required. Classes are billed to insurance. Contact your insurance if you have questions regarding insurance coverage. Our Foot Clinic Nerve damage, blood flow problems and infections can cause serious foot problems for some people with diabetes, which may result in amputations. Good foot care is an essential part of diabetes management and prevention. Early detection and proper treatment are the best ways to minimize complications. At our diabetic foot clinic, trained registered nurses offer the following services: Nail trimming. Callus removal. Education for foot care. Footwear resources. A footwear referral from your primary care provider is required along with a yearly comprehensive foot exam. Certain foot and nail conditions must be identified for insurance to cover services.
