From the two selections below, please review and complete the required tasks in both the Onboarding and Employee Health & History instructions sections.
As you begin to complete the following tasks, you may find some of the links are available only when you are logged into the internal Samaritan network.
The Employee Portal Guide and the instruction guides below provide step-by-step information on how to access and navigate PeopleSoft and symplr Performance during your onboarding process.

If you work from home or in one of Samaritan’s corporate facilities, complete the Health History Questionnaire. Email the questionnaire along with a copy of your COVID-19 vaccination record to [email protected] within 10 days of your start date.
If you work in a clinical setting, complete the following items within 10 days of your start date:
1.) Call one of the Employee Health & Safety offices to schedule your new-hire employee health screening.
2.) Complete the Health History Questionnaire and Tuberculin Screening Form. Bring a printed copy of each completed form to your employee health screening appointment or email them to [email protected].
3.) Submit your immunization records.
If you do not have documentation at the time of your appointment, the Employee Health Department will order lab testing to document immune status. If you do not demonstrate immunity, the necessary vaccinations will be provided to you at no cost.
