Samaritan Health Services is committed to protecting your health information and providing secure access to your medical records.

Use this online form to request your medical records from a past stay at one of our Samaritan hospitals or visits to any of our Samaritan medical clinics throughout Benton, Lincoln and Linn counties. Please allow 30 days to process requests for medical records. Depending on the purpose of your request, standard fees may apply for copies of your records.

Using the online medical records request form is simple, convenient and secure. This method provides medical records in formats that are preferred by patients, health care providers and other recipients.

We are dedicated to providing your records in a timely manner. Requests are processed in the order they are received. The processing time will vary, depending on the file size, location of the records and what information is being requested. Most medical records requests are fulfilled within 30 days.

You can check the status of an existing request in one of three ways:

  1. Visit ROILog.com. You will need your tracking code and request ID number.
  2. Call MRO at 610-994-7500. You can choose between an automated status check or speaking with a representative.
  3. Email MRO at [email protected].  

You can make a payment in any of the following ways:

  • Pay with major credit or debit cards at ROILog.com. You will need your tracking code and request ID number.
  • Mail a check, payable to MRO, to:

MRO is a medical records request company that handles the convenient, secure, accurate and efficient fulfillment of medical records requests for patients and health care providers alike. For patients, this means you will interact directly with MRO when you need to access your medical records.

Copies of Medical Records Are Available for:

  • Adult patients accessing their own medical records.
  • Parent or legal guardian accessing for their minor child.
  • Power of attorney with proof of legal representation.
  • Next of kin requesting medical records of a deceased family member, with proof of legal representation.

Accessing Your Records in the MyChart Patient Portal

For your convenience, you may also access your own medical record to check on things like upcoming or past appointments, lab results or immunizations through your MyChart account. If you have additional questions about MyChart, please call 541-768-5765.
