What Is Transitional Care?

Often called “Swing Bed – Transitional Care,” our Transitional Care Program provides services for patients who need skilled nursing care and/or rehabilitation before returning home after a hospitalization. 

It is a program for patients who are well enough to leave a hospital but not yet well enough to return home.

This can include people who are recovering from a major accident or stroke, who have special wound care needs, or are recovering from major surgery such as joint replacement.

The Transitional Care Team meets regularly to assess each potential candidate for this program, review program criteria and determine if the program is in the best interests of the candidate. If any of the program factors are not met, alternative placement will be discussed.

 

What Our Program Provides

Whether your original hospital stay was at Samaritan North Lincoln Hospital, Samaritan Pacific Communities Hospital or elsewhere, our caring and professional staff will ensure a safe transition from hospital to home.
You do not need to live locally to take advantage of this program and for your loved ones to enjoy the amenities of the central Oregon Coast during your stay.

What We Offer

  • Care close to home in Lincoln City and Newport instead of an out-of-area skilled nursing facility.
  • Physical Rehabilitation services including physical, occupational and speech therapy.
  • Hospital-level staffing of registered nurses and certified nurse assistants.
  • Onsite services including respiratory therapy, radiology, laboratory and pharmacy.
  • Bedside rounding with the attending physician that engages the patient, family and care team to help reach goals.
