Our Palliative Care Team

Health care professionals involved in palliative care services typically include:

  • A physician, nurse practitioner, physician assistant and/or nurse who work collaboratively with a patient’s medical team to addresses symptoms associated with serious illness. They also help patients explore their hopes and concerns, so their treatment plan matches personal goals.
  • A social worker assists patients with emotional and social issues and gets patients connected with vital resources at the hospital and in the community. 
  • A chaplain offers spiritual support for patients and loved ones according to their faith-based needs.
