Request Electronic Health Information Computer Readable Files

Request your health information in computer-readable files (files that a computer or app can read). These files are not designed to be read by humans. These files could be very large and take extra time to process. Due to the size of these files, they are only available on CD, or can be sent to your active MyChart account.

You may request and receive the export using your MyChart account. The request is located under the Sharing Hub within MyChart. Access the request by clicking Request Computer-readable Export and complete the request. If you do not have a MyChart account, you can setup a new account on the MyChart login page.

Receive Your Medical Records

Please allow us 30 days to process requests for medical records. Once our Health Information Management department receives your request, we will try to process your request within two weeks.

Fees: Standard rates apply for patient access requests that are disclosed directly to the patient or the patient representative.

CD rate: $5.00 per CD.

Standard postage rates apply for requests that are mailed.
