Host a Fundraising Event

Event Guidelines

I understand that:

• All events to benefit any of the Samaritan Health Services (SHS) affiliated foundations must be approved by the designated foundation prior to the event or its publicizing.
• You must be at least 18 years of age or older to submit a fundraising proposal. This form must be submitted at least 60 days prior to the beginning of your fundraising.
• All promotional material not provided by SHS for proposed event must be approved by SHS prior to its release.
• SHS-affiliated foundations are not permitted to advance any monies or underwrite fundraising activities.
• All necessary permits and certificates of insurance required by city ordinance or otherwise are the responsibility of your fundraising group and must be submitted with proposal for approval.
• In accordance with IRS regulations, the individual or organization conducting the fundraiser is responsible for disclosing to donors the exact dollar amount or percentage of their gift which will benefit the named benefiting foundation.
• Due to confidentiality commitments, SHS-affiliated foundations are unable to release donor, volunteer, employee, physician or other mailing lists for the purpose of solicitation of funds or participation in your event or project.
• If circumstances warrant, SHS-affiliated foundations may opt out as a beneficiary of the event/promotion at any time with no obligation.
• Your event and photographs may appear in publications and on SHS websites. Your signing of this form indicates permission to use images and your event name.
• The fundraising event or appeal organizer/sponsor/organization agrees to indemnify, refund and hold Samaritan Health Services, all affiliated hospitals and affiliated foundations harmless against and in respect of any and all claims, demands, losses, costs, expenses, obligations, liabilities damages, recoveries and deficiencies, including interests, penalties and reasonable attorney’s fees that shall be incurred or suffered by Samaritan Health Services, all affiliated hospitals and affiliated foundations which arise, result from or relate to the fundraising event or appeal, the organizer/sponsor/ organization’s performance of its agreement as specified in these guidelines and Special Event and Promotion Proposal Form.
• Within 45 days of your completed fundraising event/promotion, please send a final accounting of income and expenses along with your proceeds to the benefiting foundation listed below. Please also include contact information for individuals and organizations that need receipts for tax purposes (this includes in-kind donations). Charitable Solicitations Act 128.848.
