Join this in-person workshop where you will explore your health care decisions and have the opportunity to complete your own Advance Directive.

The Advance Directive is a set of instructions that expresses a person’s health care wishes if they were ever in the hospital and unable to speak for themselves. The Advance Directive tells the medical team who can speak for the patient and what the patient’s medical wishes are. While this document can feel overwhelming, these workshops will attempt to make the confusing feel more understandable.

Topics include:

Advance directives conversation starters.
Appointing a health care representative.
Documenting your desires in the matter of critical life support.
Brief introduction to physician orders for life-sustaining treatment (POLST).
