SUMMARY:National Diabetes Prevention Program
DESCRIPTION:The National Diabetes Prevention Program is a year-long evidence-based lifestyle change program for people diagnosed with pre-diabetes or who are at risk of developing diabetes. Participants get a full year of support and learn how to eat healthy\, add physical activity to your routine\, manage stress\, stay motivated and solve problems that can get in the way of meeting goals. \nTo participate\, you must meet all four of these requirements: \n\nBe 18 years or older.\nHave a body mass index (BMI) of 25 or higher (23 or higher if you’re an Asian American person).\nNot be previously diagnosed with type 1 or type 2 diabetes.\nNot be pregnant.\n\nYou’ll also need to meet 1 of these requirements: \n1. Had a blood test result in the prediabetes range within the past year (includes any of these tests and results): \n\nHemoglobin A1C: 5.7%–6.4%\nFasting plasma glucose: 100–125 mg/dL\n2-hour plasma glucose (after a 75g glucose load): 140–199 mg/dL.\n\n2. Be previously diagnosed with gestational diabetes (diabetes during pregnancy). \n3. Received a high-risk result (score of 5 or higher) on the Prediabetes Risk Test.
