SUMMARY:Living Well with Chronic Conditions
DESCRIPTION:Living Well With Chronic Conditions is a workshop\, in English\, for people living with on-going physical and mental health conditions like heart disease\, anxiety\, diabetes and arthritis. The skills learned will help you take charge of your health by building skills in communication\, problem solving and action planning. Gain control and feel better. \nThis is a 6-week course\, 6 total sessions for two and a half hours each week. \nAll course dates: \n\nJuly 9.\nJuly 16.\nJuly 23.\nJuly 30.\nAug. 6.\nAug. 13.\n\nTopics include: \n\nTools for action planning\, decision-making and healthy eating.\nHow to break the symptom cycle and better manage frustration\, pain\, fatigue\, isolation\, sadness and poor sleep.\nSkills to communicate better\, make informed decisions and improve problem-solving.\n\nOnce you have registered a confirmation email will be sent with further instructions. \nTo learn more about our free workshops email SHSHealthEd@samhealth.org or call 541-768-6811.
