SUMMARY:Advance Directive Workshop - Newport
DESCRIPTION:Join this in-person workshop where you will explore your health care decisions and have the opportunity to complete your own Advance Directive. \nThe Advance Directive is a set of instructions that expresses a person’s health care wishes if they were ever in the hospital and unable to speak for themselves. The Advance Directive tells the medical team who can speak for the patient and what the patient’s medical wishes are. While this document can feel overwhelming\, these workshops will attempt to make the confusing feel more understandable. \nTopics include: \n\nAdvance directives conversation starters.\nAppointing a health care representative.\nDocumenting your desires in the matter of critical life support.\nBrief introduction to physician orders for life-sustaining treatment (POLST).\n\nDrop-ins are welcome but registration is requested.
